Acting in its capacity as Data Supervisor within the scope of Personal Data Protection Law No. 6698 (“Law”) and applicable legislation, your personal data can be processed by NB Kadikoy Hospital only within the framework specified below and in accordance with the applicable legislation.
Collection of Personal Data
Your personal data processed by NB Kadikoy Hospital may vary depending on the health services provided. In addition, it is collected verbally, in writing or electronically, by call centers, websites, online services and similar instruments both automatically or non-automatically.
Your sensitive personal data, particular your health data, and your general personal data as well as the personal data listed below will be able to be processed:
- Your name, surname, T.R. identity number, passport number or temporary T.R. identity number, place and date of birth, marital status, gender, patient number specific to our hospital and other identification data by which you can be identified
- Your address, telephone number, e-mail address and similar contact data
- Your financial data such as your bank account number, IBAN number, credit card data, billing data
- Your personal data obtained when you contact us via e-mail, letter or another alternative channel as well as your voice call recordings kept by the call center
- Your private health insurance data and Social Security Institution (SSI) data for the purpose of financing and planning health services
- Your health and identity data sent by you through our websites
- Footage if you visit our hospital
- Your license plate data if you utilize parking lot of our hospital
- Your health and sexual life data obtained while conducting medical diagnosis, treatment and care services, including but not limited to your laboratory results, test results, examination data, check-up data and prescription data
- Your personal data, including your CV, health history if you apply for a job at NB Kadikoy Hospital, and all kinds of personal data related to your employment contract and competence if you are currently employed by NB Kadikoy Hospital or an affiliated employee.
- Your personal data obtained when you attend trainings, seminars or organizations organized by NB Kadikoy Hospital, or if you visit or browse the websites.

Your personal data is collected through all kinds of verbal, written or electronic channels for the purposes listed below and with the aim of providing health services within the designated legal framework and, in this regard, for NB Kadikoy Hospital to fully and appropriately fulfill its contractual and legal obligations.

- Informing you about your appointment if you made one
- Analyzing to improve healthcare services
- Planning and managing internal administration of hospital
- Analyzing to improve our services
- Billing

- Fulfilling risk management and quality improvement studies
- Conducting research
- Verifying your identity
- Obtaining medications
- Sharing the requested data with Ministry of Health or relevant state institutions and organizations in accordance with the applicable legislation
- Sharing the requested data with private insurance companies within the scope of financing of healthcare services
- Responding all kinds of questions and complaints of you regarding our healthcare services
- Taking all necessary technical and administrative measures within the scope of data protection principles of our hospital's systems and applications
- Providing necessary information with regulatory and supervisory institutions in line with the requests and inspections of official authorities
- Preserving your health data that must be kept in accordance with the applicable legislation
- Reaching a financial agreement with the contracted institutions over the healthcare services offered to you

Legal Ground and Preservation of Personal Data Collection
Your personal data can be processed within the framework of the Health Services Fundamental Law No. 3359, Decree Law Concerning the Organization and Duties of The Ministry of Health and Affiliates, Regulations on Private Hospitals, regulations of the Ministry of Health and similar legislation, and can be transferred to physical archives and information systems as well as can be preserved both in digital and physical environment.
Your personal data is meticulously protected by NB Kadikoy Hospital within our technical and administrative capabilities. Considering the technological possibilities, they are protected against possible risks in line with the security measures taken by our hospital.
Transfer of Personal Data
For the above-mentioned purposes, your personal data may be shared with the institutions or organizations listed below within the scope of the Law and similar legislation:

- The Ministry of Health and affiliate sub-units
- Social Security Institution
- General Directorate of Security Affairs and similar police forces
- Directorate General of Civil Registration and Citizenship Affairs

- Laboratory, administrative center and similar third parties with whom we cooperate for medical diagnosis
- Your authorized representatives
- Third parties from whom we receive consultancy service, including lawyers, tax advisor and auditors

Your Rights to Request Protection of Your Personal Data
- a) Learning if any of your personal data has been processed
- b) Requesting information about processing activities of your personal data
- c) Learning the purposes of processing your personal data
- d) Learning the parties with whom your personal data has been shared if transferred to any of them domestically or abroad
- e) Requesting correction of your personal data if it is incomplete or incorrectly processed
- f) Requesting the deletion or destruction of your personal data if the reasons requiring personal data processing are no longer valid or the Group does not have a legal basis or legitimate interest to process the aforementioned data
- g) Objecting to disadvantageous consequences that may arise due to processing of your personal data through automated systems
- h) Demanding compensation for damage if you sustain a loss due to illicit processing of your personal data
Your Requests Regarding Personal Data
Your request will be concluded free of charge as soon as possible within thirty days at the latest depending on characteristics of the request. However, you will be charged with a fee by the Hospital in line with the tariff specified by the Personal Data Protection Board if the process requires an additional cost.
You can submit your requests within the scope of the said law by you can hand it over in person to NB Kadıköy Hospital, Acıbadem Mahallesi, Sokullu Sokak No: 1 PB 34718 Kadikoy / Istanbul or send it through a notary public.
